Job summary

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required
• Knowledge of patient- and family-centered practice and ability to work with diverse patients and families
• Strong communication skills and ability to collaborate with the care team and patients' families
• Ability to assess patients, plan care, and educate patients and designated care partners
• Knowledge of discharge planning and post-discharge needs

Responsibilities

• Interacts with patients, families, and staff to deliver care consistent with Jefferson Health values
• Uses evidence-based practice and clinical expertise to guide patient care
• Educates patients and their designated care partners about conditions, treatments, and next steps in recovery
• Uses information technology and data to monitor care outcomes and improve safety and quality
