Job summary

Jefferson Health is seeking a dedicated registered nurse to join our team in a patient- and family-centered care environment. You will develop nursing plans of care with patients and their designated care partners, ensuring care aligns with cultural beliefs and values.

You will collaborate with inter-professional teams, apply evidence-based practice, and educate patients and families about care plans, medications, and post-discharge steps.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Active RN license in state of practice required.
• Knowledge of patient, family and care partner collaboration in care/planning.
• Knowledge of cultural diversity and respectful care.
• Understanding of effective communication concepts and techniques.
• Ability to delegate appropriately within the care team.
• Knowledge of patient assessment, care planning, and discharge needs.
• Familiarity with health information documentation and basic data handling.
• BLS and AHA certifications for nursing practice.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partners, and other staff consistent with Jefferson values.
• Integrates Evidence-Based Practice and clinical expertise with patient preferences for optimal care.
• Partners with patients and families to identify learning needs and provide understandable education.
