Qualifications

• Enrolled in an accredited program within 24 months of employment; must obtain BSN within five years.

Responsibilities

• Assess physiologic health status (cardiovascular, GI, respiratory, renal, neuro).
• Assess psychosocial health status and coping mechanisms; identify cultural needs.
• Develop and update the plan of care with priorities and outcomes.
• Coordinate care with physicians and health team; document plan and actions.
• Implement treatments and monitor patient safety; adjust as needed.
• Educate patients and families; provide discharge planning and referrals.
