Job summary
BAYADA Home Health Care in Jacksonville, FL is seeking a Registered Nurse (RN) for home health visits. The RN will provide compassionate, high-quality in-home care to adult and geriatric clients across designated territories.
The role requires a Florida RN license, at least 1 year of clinical experience, and willingness to travel. Home health experience is preferred; strong interpersonal and time-management skills are essential.
Qualifications
- Active Florida RN license is required.
- Minimum 1 year of recent, verifiable clinical (medical/surgical) experience.
- Home health experience preferred, but not required.
- Graduation from an accredited nursing program.
- Ability to travel to assigned cases.
Responsibilities
- Follow designated care plans for patients in assigned territories.
- Make home visits to clients in Jacksonville, FL.
- Administer medications, wound care, treatments and procedures.
- Monitor patient conditions and report changes.
- Document observations and care in the EMR.
- Coordinate with Clinical or Client Services Manager.
- Perform client assessments as needed.
- Case management and coordination.
