Job summary

Enhabit Home Health & Hospice in Jacksonville, FL is seeking a licensed nurse to administer skilled nursing services in the home and educate patients and families on the care plan.

You will collaborate with physicians, coordinate with care teams, and ensure treatment aligns with orders and the established POC. Valid state license and CPR are required; driver license and reliable transportation are essential.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed nurse.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled nursing services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
