Job summary

COLUMBINE WEST HEALTH & REHAB FACILITY in Fort Collins, CO is seeking a dedicated Registered Nurse to provide direct patient care, develop care plans, administer medications, and monitor patient status in a fast-paced clinical setting.

The RN will collaborate with physicians and the interdisciplinary team, document care per facility standards, and support nursing staff while prioritizing patient safety and compassionate, person-centered care. This is a full-time, on-site role.

Qualifications

• Active RN license in Colorado and graduation from an accredited nursing program.
• Clinical skills in patient assessment, care planning, medication administration, and wound care.
• Knowledge of rehabilitation, long-term care, or acute care practices and relevant regulations.
• Strong communication, collaboration, and interpersonal skills for working with patients, families, and interdisciplinary teams.
• Ability to prioritize tasks, manage time effectively, and respond to changing patient needs.
• Commitment to patient safety, infection control, and quality improvement initiatives.
• Basic computer and electronic health record documentation skills.
• Previous experience in a hospital, skilled nursing, or rehab setting is preferred but not required.
• Ability to meet physical demands of the role, including standing, lifting, and assisting with patient mobility.

Responsibilities

• Provide direct patient care and daily assessments.
