Job summary

The Paramount at Somers Rehabilitation and Nursing Center is seeking Registered Nurses (RNs) to join a compassionate, skilled team in Somers, NY. You will provide high-quality clinical care and play a vital role in residents’ day-to-day health outcomes.

Qualifications

• Current and active NY RN license.
• Experience in SNF/rehabilitation/long-term care preferred; new graduates welcome.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Perform comprehensive assessments and monitor changes in residents' conditions.
• Administer medications, treatments, and IV therapy as prescribed.
• Coordinate resident care with physicians and the interdisciplinary team.
• Supervise and support LPNs and CNAs.
• Maintain accurate clinical documentation in the EMR.
• Respond to medical emergencies and changes in resident condition.
• Ensure compliance with all policies and regulations.
• Promote a safe, respectful, and resident-centered environment.
