Job summary

Northwell Career Site is seeking an experienced Registered Nurse to join our clinical team. The role focuses on delivering high-quality patient care across physiologic and psychosocial domains, assessing health status, planning care, and coordinating with physicians.

You will implement the care plan, monitor outcomes, educate patients and families, document care, and collaborate with the care team to ensure safe, effective discharge planning. Requires NY/CT RN license and BLS.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing, required.
• Bachelor’s Degree in Nursing, preferred.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut, required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses the patient’s physiologic health status.
• Assesses patient’s psychosocial health status.
• Identifies patient outcome.
• Formulates the plan of care and establishes the priorities necessary to achieve expected outcome.
• Implements the plan of care.
• Evaluates care provided for patient outcome.
• Collaborates with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team.
• Performs grade I-IV Decubitus Care.
• Administers medications correctly and safely.
• Participates in patient and family education.
• Performs related duties, as required.

Education

Associate’s Degree in Nursing

Bachelor’s Degree in Nursing (preferred)

Current RN license in New York State and/or Connecticut

BLS certification
