Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic in-home care to patients and families, partnering with an interdisciplinary team to develop individualized care plans and guide end-of-life conversations.

Requirements include an accredited nursing degree and 1+ years of acute care experience; hospice/home health experience is preferred. The position offers a full-time schedule with on-call duties and a comprehensive benefits package.

Qualifications

• Associates, Bachelors, or Masters in an accredited nursing program (RN).
• 1+ years of acute care experience required; hospice/home health experience preferred.
• Demonstrated ability to develop individualized care plans and educate patients and caregivers.
• Ability to participate in on-call duties and coordinate seamless transitions of care.

Responsibilities

• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Develop individualized care plans with patient and family, updating as needs evolve.
