Job summary

Visiting Nurse Health System is hiring an RN Case Manager for Home Health in Marietta, GA. You will coordinate care for a caseload of patients, ensuring quality standards and utilization guidelines are met, with focus on patient education and family engagement.

You will assess, plan, implement, monitor, and evaluate care plans, work with an interdisciplinary team, and deliver compassionate, home-based nursing care while upholding VNHS policies and nursing standards.

Qualifications

• Graduated from an accredited nursing program (Diploma/ADN/BSN).
• GA State Board of Nursing licensure with CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Prior home health experience preferred.
• Knowledge of Medicare/Medicaid and related funding.
• Strong clinical nursing skills and team collaboration.
• Independent decision making and time management in home settings.
• Proficient with computers for documentation and training.

Responsibilities

• Develop and implement care plans per physician orders with the team.
• Modify care plans as patient conditions change and assess effectiveness.
• Educate patients and families on disease processes and self-management.
• Complete and submit OASIS, visit notes, and required records on time.
• Communicate patient status changes to patients, physicians, and team.
• Offer emotional support and connect with MSW and community resources.
• Supervise LPNs and HHAs per VNHS policy.
• Participate in SOC/ROC calls and weekly team conferences.
• Deliver high-quality care in the home following VNHS policies.
• Maintain competencies through in-services and education.
• Schedule patient visits and manage caseload for productivity.
• Precept newly hired nursing staff and coordinate with office staff.

Job Overview

Visiting Nurse Health System is hiring an RN Case Manager for Home Health who is responsible for coordinating care delivered to a caseload of patients, ensuring care meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure optimal patient outcomes. This position requires a compassionate and skilled nurse who thrives in a home-based environment and values patient education and family engagement.

What You’ll Do:

• Develop and implement individualized plans of care in accordance with physician orders by assessing patient needs and collaborating with the interdisciplinary team.
• Modify and update care plans as patient conditions change, regularly evaluating their effectiveness to ensure ongoing effectiveness.
• Provide patient and family education on disease processes, self-management strategies, and methods to prevent complications, such as hospitalizations.
• Complete and submit OASIS documents, visit notes, and other required records within VNHS timeliness guidelines.
• Communicate changes in patient status clearly to patients, physicians, and team members, and document them appropriately.
• Offer emotional support and connect patients/families with MSW and other community resources as needed.
• Supervise LPNs and HHAs in accordance with VNHS policy, ensuring that care plans are in place and meet patient needs.
• Participate in SOC/ROC conference calls for new or resuming patients, as well as in weekly team conferences.
• Deliver competent, high-quality care in the home, adhering to VNHS policies and nursing standards.
• Maintain required competencies through in-services and continuing education.
• Schedule patient visits efficiently in accordance with orders and manage case load to optimize productivity.
• Precept newly hired nursing staff and communicate effectively with the team and office staff regarding patient status or concerns.

You’re Perfect for the Position if you:

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
• Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
• Possess a valid driver’s license, reliable transportation, and current auto insurance.
• Have prior home health experience if applying as a PRN staff member.
• Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to home health care.
• Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
• Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment.
• Are proficient with computers and basic technology for documentation, email, HR systems, and online training.
