Job summary

Ambercare Hospice in Belen, NM is seeking a compassionate PRN Registered Nurse Case Manager to provide expert clinical care and support to patients and families during meaningful moments. You will coordinate visits, manage caseloads, and collaborate with the interdisciplinary team to ensure comfort and quality of life.

The role emphasizes autonomy, patient-centered care, and opportunities to grow within a mission-driven hospice team while delivering compassionate services across the community.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment and care planning.
• Obtain data from patient physical, psychological, social, and spiritual factors.
• Initiate communication with the attending physician and interdisciplinary team for coordinating care.
• Provide regular visits to reassess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide family support.
• Maintain accurate patient records and supervise Home Health Aide delivery of care.
