AccentCare, Inc. is seeking a compassionate RN Case Manager to oversee patient care plans in home health across Madison, Oconee, Clark and Walton Counties in Georgia. This role combines direct patient care with supervision of nursing staff to ensure quality outcomes.

You will assess patient needs, coordinate treatments, and document progress from admission to discharge while maintaining CPR certification and RN licensure. Competitive compensation is provided with full-time hours.

Qualifications

• RN must be a graduate from an approved school of nursing
• Currently licensed as an RN in the state of operation
• CPR certification required

Responsabilités

• Manage patients' care plans from admission through discharge
• Direct staff nurses, LPNs, and CHHAs in delivering the plan of care
• Assess nursing needs and establish measurable goals
• Coordinate medications and treatment plans
• Document assessments, progress, and discharge notes timely
