Job summary

Elite Home Health & Hospice in Lewiston, Idaho is seeking a dedicated Registered Nurse Case Manager to coordinate and deliver high-quality home health and hospice services. You will complete comprehensive patient assessments, develop care plans, and collaborate with physicians and therapists to support patients and families in their homes.

Qualifications

Idaho RN license in good standing.
Graduate of an accredited nursing program.
One year of nursing experience required.
Home health or hospice experience preferred.
Strong assessment, communication, and organizational skills.
Ability to work independently and manage a caseload.
Current driver's license and reliable transportation.

Responsibilities

Complete comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Provide skilled nursing care per physician orders and agency policies.
Monitor patient progress and modify care plans as needed.
Educate patients and families on disease management, medications, and plan of care.
Maintain accurate and timely clinical documentation.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with federal, state, and agency regulations.
