Job summary

Easter Seals NH seeks a compassionate Nurse Case Manager (RN) to coordinate nursing care within our busy Adult Day Program in Manchester, NH. You will collaborate with health care providers to manage medications, treatments, and daily care plans, ensuring safety and independence for clients.

The role requires NH RN licensure (or multi-state compact) with at least 1 year of relevant experience; CPR/First Aid certification preferred. Full-time schedule with a benefits package is offered.

Qualifications

• RN license in New Hampshire or multi-state compact license.
• 2+ years of experience in a health-related field preferred.

Responsibilities

• Coordinate communication between physicians, clients, families and staff.
• Document progress through nursing notes and medication/treatment records.
• Develop and update client treatment plans and conduct medical re-certifications as required.
• Obtain past medical records and ensure compliance with protocols.
• Provide dietary counseling and nutritional assessments as indicated.
• Maintain licensure, confidentiality and high professional standards.
