Job summary

Easterseals NH is seeking a part-time Nurse Case Manager (RN) for its Adult Day Program in New Hampshire. The role includes nursing care procedures, medication administration/supervision, assessments, and coordinating with health care providers to support client independence during program hours.

Responsibilities include updating care plans, medical re-certifications, obtaining medical records, and documenting client progress.

Qualifications

• RN licensed in New Hampshire or multi-state compact license.
• CPR and First Aid certification required.
• Bachelor’s or Associate degree with health-related field experience.

Responsibilities

• Coordinate medical care and medications for assigned clients.
• Maintain nursing progress notes and treatment plans.
• Obtain past medical records and document client status.
• Provide preventative health education and dietary counseling.
• Ensure proper medication administration and nursing duties under physician direction.
