Job summary

Easterseals NH in Manchester, NH is seeking a compassionate Nurse Case Manager (RN) for our busy Adult Day Program. You will coordinate nursing care, assessments, and emergencies, and work with health-care providers to ensure medication and treatment compliance.

This full-time role offers 35-40 hours weekly, Monday–Friday between 7:30am–5pm, with a salary range of $63,700–$72,800 and a benefits package including medical, dental, vision, PTO, and retirement match.

Qualifications

• Bachelor’s Degree from an accredited institution plus two years in a health-related field, or an Associate’s Degree with four years in a health-related field.
• A Registered Nurse licensed in New Hampshire, or a multi-state compact RN, with at least one year in a health-related field.
• CPR and First Aid certification, valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Coordinate communication among physicians, clients, families and staff on medical issues.
• Develop and update each client’s master problem list and treatment plan; complete medical re-certifications.
• Obtain past medical records from hospitalizations for new clients.
• Attend staff meetings, intake meetings, case conferences and family meetings.
• Document progress, medications, treatments and client status daily.
• Review and sign medical orders; provide physician-ordered services under supervision.
• Conduct nutritional profiles and provide diet counseling as required.
• Provide health education to groups/individuals on preventive topics.
• Provide temporary emergency medical care until EMS arrives.
• Order supplies and maintain medications in safe, labeled form.
• Assist with daily social/leisure programming as needed.
• Maintain professional ethics and positive inter-agency relationships.
• Ensure safety and infection control; uphold licensure and continuing education.

Job Description:

We are seeking an easy going, multi-tasking, compassionate, team oriented Nurse Case Manager (Registered Nurse) for our busy and active Adult Day Program at Easterseals NH! The individual in this key position will be responsible for all nursing care procedures and related health care activities, assessments, personal care activities and emergencies for aging individuals. The Nurse Case Manager will work closely with each individual’s health care providers to ensure compliance with medications/treatment protocols and to assist individuals to maximize his/her independent level of function.

Schedule: Full-time 35-40 hours a week; Monday-Friday flexible hours between 7:30a-5p

Compensation: $63,700 - $72,800

Additional Job Responsibilities:

• Coordinate communication between Physicians, inter/intra agency, clients, families and staffin regard tomedical issues. Keep team advised as to any changes in assigned client’s medical status, precautions, and/or treatment protocols.
• Develop, update, and carry out assigned client’s master problem list and program treatment plan as required. Complete medical re-certifications on all assigned clients per program procedure.
• Obtain past medical records from previous hospitalizations on all new clients.
• Participate in staff meetings, intake meetings, assigned case conferences and schedule/attend family meetings on assigned clients as required.
• Document assigned client’s progress through nursing progress notes as defined in Agency Policy Manual. Record daily medications, treatment, personal care and client’s physical status on appropriate medication and assessment sheets.
• Ensure that all medical orders are properly reviewed, documented and signed for on assigned clients. Provide physician ordered services under the direction of a physician.
• Conduct nutritional profiles on all clients and provide dietary counseling as indicated. Obtain written doctor’s order for all special diets.
• Facilitate group/individual health education around preventative and informational areas.
• Provide temporary emergency medical care until appropriate EMS personnel arrive.
• Order medical supplies as needed. Maintain medication in safe/clean order. Ensure that medications are either ordered appropriately or necessary medications are brought in from home in original, correctly labeled prescription bottles.
• Assist with daily social/leisure programming, as needed.
• Maintain professional ethics, appearance, attitude and conduct. Maintain positive working relationship with both inter/intra agency sources.
• Help to ensure and maintain the cleanliness and safety of program areas. Follow Safety and Infection Control Policies at all times.
• Maintain state and professional requirements for licensure/certification and continuing education. Assume responsibility for professional growth.
• Maintain client confidentiality and treat all clients with respect.
• Perform additional duties as requested.

Qualifications:

• A Bachelor’s Degree from an accredited institution and at least two (2) years of experience working in a health related field; orAn Associate’s Degree from an accredited institution plus four (4) years of experience in a health related field
• A Registered Nurse (RN), licensed in New Hampshire, or a Registered Nurse with a multi-state compact license, with at least one (1) year of experience working in a health related field
• CPR and First Aide certified
• Valid driver’s license, auto insurance, and reliable transportation
