Job summary

Human Coalition in Plano, TX is seeking a full-time RN Nurse Care Coordinator to deliver in-clinic and telecare assessments, education, and support for women facing unplanned pregnancies. You will leverage nursing expertise to assist our mission of rescuing children and supporting families across America.

This onsite role reports to the Practice Manager and involves collaboration with a multidisciplinary team, thorough documentation in the CMS, and adherence to HIPAA and safety protocols while

Qualifications

• Must hold a current RN license in good standing and a compact license if applicable.
• CPR certification current and up to date for healthcare providers.
• 2–4 years of clinical experience preferred in obstetrical/gynecological care.
• Strong HIPAA knowledge and patient confidentiality emphasis.

Responsibilities

• Evaluate client situations over the phone to address chief complaints or concerns.
• Provide education on abortion, parenting, and adoption services by phone and in person.
• Connect clients with resources to relieve pressure and support decisions.
• Document interactions and care plans accurately in the CMS.
• Offer counseling, education, and emotional support to clients and families.
• Perform limited medical services as appropriate and safe onsite.
