Job summary

Human Coalition in Grapevine, TX is seeking a Nurse Care Coordinator (LVN/RN) for a full-time, on-site role. You will provide face-to-face in-clinic and telecare assessments, education, and support to clients facing unplanned pregnancies, guided by compassionate, patient-centered care.

Qualifications

• Must hold LVN or RN license and be in good standing with the state board.
• 2-4 years of clinical experience in any specialty (preferred).
• Emphasis on obstetrical and gynecological care preferred.
• Must have current CPR certification for healthcare providers.

Responsibilities

• Evaluate a client’s situation over the phone to identify concerns and plan care.
• Provide education on abortion, parenting, and adoption services.
• Connect women with resources to relieve pressures and support decisions.
• Document all client interactions and care plans in the CMS.
• Perform limited medical services and support the Medical Director as needed.
• Collaborate with the medical team to ensure safe, confidential care.
