Job summary

Diversicare is seeking a Certified Nurse Assistant (C.N.A.) to support residents in a skilled nursing environment in Oak Ridge, TN. The role focuses on safe patient transport, accompanying residents to medical visits, and ensuring all documentation is accurate and complete.

Responsibilities include assisting with transfers, documenting visits, and following provider orders while adhering to Diversicare Service Standards and core values. A compassionate, communicative caregiver is essential.

Qualifications

• Certified Nurse Assistant (C.N.A.) with valid state certification.
• Physically able to assist ambulatory and non-ambulatory residents transfer.
• Possess a valid state driver’s license and clean driving record.
• Clean criminal record and any required state certifications.

Responsibilities

• Assure the safety of each resident when transporting to/from doctor’s office or hospital.
• Accompany each resident on all medical visits and stay with them until visit completion.
• Observe required check-in and documents at visits.
• Obtain resident’s documents and new prescriptions before leaving the doctor’s office; verify orders.
• Obtain appointment cards if follow-up is necessary.
• Perform other duties as required and assigned.
