Job summary

Patriot Health and Rehabilitation Center – Paris, TN is seeking a dependable Van Driver to join our skilled nursing team. This full-time role transports residents to medical appointments and provides compassionate care throughout transportation.

Requirements include a valid Class F endorsement or driver’s license, active TN CNA, and a safe driving record. The role supports residents with transfers and mobility while working with the nursing team.

Qualifications

• Must possess a valid Class F endorsement or driver’s license.
• Active CNA in the state of Tennessee.
• Valid driver’s license with a safe driving record.
• Ability to assist residents with transfer and mobility.
• Reliable, dependable, and punctual.
• Ability to work independently while maintaining communication with the nursing team.

Responsibilities

• Safely transport residents to and from medical appointments, procedures, and other approved destinations.
• Assist residents in safely entering and exiting the facility vehicle.
• Provide assistance with wheelchairs, walkers and other mobility devices.
• Ensure residents are comfortable and secure during transportation.
• Maintain a clean, safe, and well-maintained facility vehicle.
• Follow all facility transportation policies and applicable safety regulations.
• Communicate appointment details, delays, concerns or changes with facility staff.
• Assist residents with basic CNA duties as needed before and after transportation.
• Complete required transportation documentation accurately.
• Perform other duties as assigned.
