Job summary

Diversicare Healthcare Services, LLC in Birmingham, AL is seeking a Certified Nurse Assistant (CNA) to provide compassionate care and assist residents with daily activities. You will ensure safe transfers, accompany residents to medical visits, and uphold our Service Standards to support well-being and dignity.

Ideal candidates are licensed CNAs with a clean driving record, valid state license, and commitment to teamwork, safety, and respectful communication within a post-acute setting.

Qualifications

• Certified Nurse Assistant (C.N.A.) required.
• Valid state driver’s license and clean driving record.
• Able to assist with transfers and mobility.
• Clean criminal record and any other state required certifications.

Responsibilities

• Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.
• Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.
• Observe the following guidelines at each visit.
• Sign in resident upon entrance.
• Present resident’s documents to office personnel.
• Before leaving doctor’s office, please observe the following:
• Obtain resident’s documents.
• Obtain any new prescriptions.
• Check to make sure doctor has written on the doctor’s order form.
• If follow-up appointment necessary, obtain appointment cards.
• After returning to facility, report to RCM and be prepared to answer questions concerning visit.
• Performs other duties as required and assigned.
