Job summary

Axiom Care in Phoenix, AZ is seeking a Driver to coordinate with the shift lead and connect clients with the services they require. As the first point of contact, you will interact with clients professionally and with compassion, transport them to Axiom facilities, and orient new arrivals to housing staff.

You will de-escalate crises when needed, leverage recovery experience to encourage engagement, and ensure warm hand-offs to receiving facilities.

Qualifications

• HS diploma or equivalent required; behavioral health experience preferred.
• Must have a valid AZ driver’s license with a clean MVR and be able to drive 15-passenger van.
• CPR/First Aid certification and basic computer skills required.
• Ability to read and interpret policies, plus strong communication and teamwork.

Responsibilities

• Pick up clients from hospitals, police stations, jails, and referral partners and transport to the appropriate Axiom facility.
• Help orient new clients to the facility and introduce them to housing staff on arrival.
• De-escalate crisis situations as needed and use recovery experience to encourage participation in treatment.
• Safely transport clients to and from off-site appointments and ensure warm hand-offs.
• Outreach to referral partners and assist with client intake, EMR onboarding, and front-desk duties as needed.
