Job summary

MorningStar Senior Living Fort Collins is hiring a Med Care Manager/QMAP for full-time hours. You will administer, document, and manage medications under physician orders, following the five rights and state regulations.

The role requires timely ordering and receiving meds, and documenting administration in residents' records with accuracy and care. The position emphasizes compassionate care, privacy, and daily support for residents, with a culture rooted in servant leadership and core values

Qualifications

• Must have completed state-approved Medication Administration training
• QMAP certification in CO is required
• Minimum high school diploma; healthcare experience preferred

Responsibilities

• Administer and document routinely prescribed medications per physician orders.
• Order meds from pharmacy to ensure availability; notify Wellness Director if not received within 24 hours.
• Document medications after administration with date and initials.
• Observe and report reactions/side effects to the licensed nurse.
• Take and record vitals prior to medications that affect vitals.
• Report medication errors to the Wellness Director immediately.
• Assist with care plan development and resident privacy and dignity.
• Assist with daily living activities and observe intake of food and care needs.
• Demonstrate core values: Love, Kindness, Honesty, Goodness, Fairness, Respect.
