Job summary

You will deliver therapeutic exercises, implement home safety interventions, travel to patient homes, and document treatments in an EHR while collaborating with the care team.

Qualifications

• Current Physical Therapy Assistant licensure/certification.
• Strong clinical skills in therapeutic exercise, mobility, and safety-focused care.
• Excellent communication, teamwork, and empathy for patients and families.
• Proficiency with EHR documentation and attention to clinical detail.
• Ability to travel and provide care within patients’ homes.

Responsibilities

• Provide home-based PT under supervision of a licensed Physical Therapist.
• Deliver therapeutic exercises and mobility training.
• Implement home safety interventions to support recovery and independence.
• Travel to patient homes and deliver hands-on care.
• Document patient treatments and progress in an electronic health record (EHR).
• Collaborate with an interdisciplinary care team to support coordinated outcomes.
