Responsibilities

• Assesses and documents patient's medical, mental and emotional needs at admission and ongoing.
• Develops and implements a care plan including discharge planning.
• Acts as patient advocate and provides compassionate, timely care.
• Demonstrates clinical reasoning and documents actions and plan of care.
• Identifies normal/abnormal course of illness and intervenes appropriately.
• Communicates patient information and documents therapist actions and plan.
