Qualifications

• Assesses and documents patient's medical, mental and emotional needs at admission and on-going.
• Develops and implements a plan of care including discharge planning.
• Advocates for patients and families with compassionate, timely care.
• Demonstrates clinical reasoning and evaluates therapy interventions.
• Identifies deviations from the normal course of illness and intervenes.
• Communicates and documents the plan of care clearly.

Responsibilities

• Acts as patient advocate and delivers care in a safe, timely manner.
• Performs clinical assessments and evaluates therapy interventions.
