Job summary

Adoration Health is seeking an experienced Behavioral Health Home Health Nurse to provide direct and indirect patient care in home or alternative settings. You will assess patients, develop care plans with physicians, and perform skilled nursing interventions per physician orders.

The role includes educating patients and caregivers, supervising LPNs and home health aides, and ensuring documentation meets regulatory and payer standards.

Qualifications

• RN with a master’s degree in psychiatric/mental health nursing and licensed in the practice state.
• RN with a bachelor’s degree in nursing and licensed in the state; 1 year of recent nursing experience preferred.
• RN with a diploma or Associate degree and licensed in the state; 2 years of recent nursing experience preferred.

Responsibilities

• Perform initial assessment and develop patient care plan with physician and Clinical Manager.
• Manage medical illnesses per plan of care; perform psycho-biological interventions.
• Conduct comprehensive medication reviews and educate patients and caregivers on regimens, side effects, and interactions.
• Provide education to patients and families on medication administration and safety.
• Administer intramuscular or intravenous medications as prescribed.
• Provide counseling and psychoeducation to patients and families.
• Document care and ensure compliance with state, payer, and agency requirements.
• Collaborate with a social worker to coordinate counseling services as needed.
• Supervise LPNs and home health aides when indicated and ensure quality of care.
