Job summary

UnitedHealth Group is seeking a PRN Physical Therapist for Home Health in North Carolina. You will perform assessments, establish treatment plans, and deliver interventions to improve mobility, endurance, and overall well‑being, while coordinating with the health care team.

Responsibilities include evaluating functional status, developing the plan of care, documenting progress, and ensuring care aligns with state guidelines. Excellent patient communication is essential.

Qualifications

• Active Physical Therapy license in the state of practice.
• Valid driver’s license and access to a dependable vehicle or reliable transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Evaluates patients' functional status and PT needs; assists physician in plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients and families about plan of care, limitations, and progress.
