Job summary

Juniper Gardens Center for Nursing and Rehabilitation in Gastonia, NC is seeking a dedicated Physical Therapist for PRN hours. You will evaluate patients, develop individualized treatment plans, and provide hands-on therapy tailored to resident needs.

Collaborate with physicians, nurses, and the interdisciplinary team; supervise PTAs and students; maintain documentation and stay compliant with regulations. NC license eligibility is required to join our caring, growth‑oriented therapy team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Must hold or be eligible for a North Carolina Physical Therapy license.

Responsibilities

• Evaluate patients in accordance with physician referrals
• Develop individualized treatment plans with functional, goal-driven outcomes
• Deliver direct, hands-on therapy tailored to resident needs
• Provide discharge recommendations and instruct families on home programs
• Communicate regularly with physicians, nursing staff, and interdisciplinary team members
• Attend care conferences and participate in quality improvement initiatives
• Supervise PTAs, rehab techs, and students
• Maintain timely and accurate documentation that meets all regulatory requirements
• Track treatment charges following billing guidelines
• Stay current with Medicare and insurance-related regulations
