Job summary

Juniper Gardens Center for Nursing & Rehabilitation in Gastonia, NC is seeking a dedicated Part-Time Physical Therapist to join our rehabilitation team. You will evaluate patients, develop and implement individualized plans, and work closely with an interdisciplinary staff to restore mobility and independence.

This role emphasizes documentation, discharge planning, patient education, and supervision of aides and students. Travel between nearby facilities is possible to meet patient needs.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license or eligibility for licensure in the state where services are provided.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of an interdisciplinary team.
• Strong organizational and documentation skills.
• Knowledge of Medicare and other applicable reimbursement requirements.
• Commitment to patient-centered care and positive clinical outcomes.

Responsibilities

• Evaluate patients based on physician referrals and clinical needs.
• Develop individualized treatment plans with functional outcomes.
• Provide treatment per plan of care and professional standards.
• Monitor progress and modify treatment plans as clinically appropriate.
• Maintain accurate and timely clinical documentation and billing guidelines.
• Educate patients, families, and nursing staff on therapies and home exercise programs.
• Participate in discharge planning and transitions of care.
• Communicate with interdisciplinary team regarding patient progress and plans.
• Supervise PTAs, therapy students, and rehab staff as appropriate.
• Travel or float between nearby facilities as needed.
