Job summary

Empath Health's Empath Home Health is seeking a compassionate PRN Home Health RN to deliver individualized care to patients in north Hillsborough County, including Lutz, New Tampa, Tampa Palms, Temple Terrace, University, and surrounding areas.

Schedule includes 10+ home health visits per week with required weekend/holiday on-call rotations. Join a mission-driven team dedicated to Full Life Care for All and potential growth to full-time as the market expands.

Qualifications

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
