Zusammenfassung

Empath Home Health, part of Empath Health, is seeking a compassionate PRN Home Health RN to join our team delivering individualized care to patients in their homes in north Sarasota county. Expect 10+ home health visits per week with weekend/holiday on-call rotations.

You will perform comprehensive assessments, develop plans of care, provide skilled nursing, and educate patients and families, collaborating with a multidisciplinary team to ensure holistic care.

Qualifikationen

• Graduate of an approved nursing program with an active Florida RN license.
• Prefers BSN or MSN degree.
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred.
• Strong assessment, critical thinking, and patient education skills.
• Willingness to participate in weekend and holiday on-call rotations.

Aufgaben

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled nursing services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
