Qualifications

Nursing degree and Illinois licensure required.
Minimum of one year in healthcare, geriatric care preferred.

Proficient in basic computer skills (Word).

Responsibilities

Manage medication storage and labeling per Illinois Administrative Code.
Ensure medications are administered and documented per procedures.
Respond to medical emergencies and document events.
Document all medically related activities.
Collaborate with family caregivers and medical professionals.
Develop care plans with Site Manager and family.
Update participant files with changes in status and contact details.
Review care plans semi-annually or as health status changes.
Maintain health history, progress notes, medication sheets, and care plans.
Monitor medical status and inform Site Manager and caregivers of changes.
Assess medical, cognitive, and functional status on admission and after hospitalization.
Provide health screenings and advise on significant changes.
Maintain inventory of health-related supplies.
Supervise program assistants and volunteers.
Provide in-service to staff as requested.
Perform additional health-related activities per policy.
Report elder abuse/neglect to Illinois Department on Aging.
Contribute to positive culture and mission.
