Job summary

St. John’s Community Care is seeking a PRN Program Nurse to support an Adult Day Program. You will manage medications, document procedures, respond to emergencies, and develop individualized plans of care with the Site Manager and family caregivers.

You will supervise staff and volunteers, coordinate with other medical professionals, and ensure compliance with Illinois Administrative Code. This role requires flexibility and a collaborative approach to resident well-being.

Qualifications

• Bachelor of Science in Nursing or RN/LPN under RN supervision.
• Licensed in Illinois and malpractice insurance.
• Minimum of one year experience in health care.

Responsibilities

• Manage locked storage and labeling of medications for administration during program hours.
• Assure medications are administered and documented appropriately.
• Respond to medical emergencies and document procedures.
• Document all medically related activities.
• Work with family caregivers to communicate with medical professionals.
• Develop plan of care with Site Manager, families, and staff.
• Update participant files for changes in status, contact details, and needs.
• Review and update plan of care semi-annually or as health status changes.
• Maintain health history, progress notes, medication sheets, and care plans.
• Monitor medical status and inform Site Manager of changes.
• Assess medical, cognitive, and functional status on admission and after hospitalization.
• Provide health screening programs and advise on changes.
• Maintain inventory of health supplies and equipment.
• Supervise staff and volunteers.
• Provide in-service to staff as requested by Program Director.
• Perform additional health-related activities as required by policy.
• Report elder abuse/neglect to Illinois authorities as mandated.
• Contribute to positive culture and mission.
