Job summary

Unity Hospice WI is seeking a PRN Certified Nursing Assistant for the Day Shift. The CNA provides delegated patient care under the supervision of an RN, following the plan of care and supporting activities of daily living.

This role requires completing a CNA program, being listed on the Wisconsin Nursing Assistant Registry, and having a valid driver’s license with reliable vehicle access. Shifts include day hours and may include weekends/holidays as needed.

Qualifications

• Completion of a certified nursing assistant program.
• Must be listed on the Wisconsin Nursing Assistant Registry.
• Valid driver’s license, auto insurance, and access to a reliable vehicle.

Responsibilities

• Provide CNA services under the supervision of an RN per the assessment and plan of care.
• Assist with Activities of Daily Living, mobility, nutrition and safety.
• Take vital signs, document findings, and report changes to the RN promptly.
• Maintain confidentiality and comply with Unity policies and regulations.
• Share in providing 24/7 coverage and be flexible with shifts as needed.
