Job summary

Desort is seeking a dedicated Physical Therapist (PT) to provide rehabilitative care, improve mobility, and enhance quality of life for patients in a clinical setting.

You will assess patients, develop plans, supervise assistants, coordinate with physicians, document progress, and contribute to a patient-centered team. This role requires licensure in Florida and a PT degree, with strong communication and interdisciplinary collaboration.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license or eligibility for licensure in the state of practice.
• Strong clinical assessment and treatment planning skills.
• Experience with rehabilitation documentation and patient progress tracking.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary team.
• Experience in rehabilitation, long-term care, skilled nursing, or geriatric settings preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on clinical findings and rehabilitation goals.
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.
• Monitor patient progress and modify treatment plans as needed.
• Establish realistic and achievable rehabilitation goals.
• Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely.
• Educate patients and family members regarding treatment plans, exercises, and recovery strategies.
• Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care.
• Supervise and support Physical Therapist Assistants as appropriate.
• Maintain compliance with professional standards, regulatory requirements, and patient confidentiality guidelines.
• Promote a safe, positive, and patient-focused care environment.
• Perform additional duties as assigned.
