Job summary

Berkshire Health & Rehabilitation Center in Vinton, VA is seeking a Physical Therapist to join our rehabilitation team and provide hands-on care that supports independence and improved function for our residents.

You will evaluate patients, develop individualized plans, supervise PTAs, document progress, and collaborate with an interdisciplinary team to achieve rehabilitation goals while educating residents and families on home exercise and safety post-discharge.

Qualifications

• Active unencumbered licensure as a Physical Therapist in Virginia.
• Degree in Physical Therapy from an accredited program.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

Responsibilities

• Evaluate residents to assess physical function, mobility, strength, balance, and rehabilitation potential.
• Develop and implement individualized treatment plans based on each resident’s plan of care and physician orders.
• Administer therapeutic exercises, manual therapy, gait training, and other prescribed interventions to restore function and reduce pain.
• Supervise, coordinate, and direct the work of PTAs.
• Explain procedures and treatments to gain residents’ cooperation and reduce apprehension.
• Monitor and document resident progress, modifying treatment plans as needed to achieve outcomes.
• Maintain residents’ medical records with thorough documentation of evaluations, treatments and progress.
• Collaborate with the interdisciplinary team to ensure continuity of care and rehabilitation goals.
• Educate residents and families on home exercise programs and safety post-discharge.
• Recommend appropriate assistive devices and durable medical equipment as needed.
