Job summary

Eastern Health Group in Lynchburg, Virginia is seeking a licensed Physical Therapist to assess and treat residents in our skilled nursing and rehabilitation centers.

You will collaborate with the rehabilitation team to design treatment plans, set goals, document progress, and educate patients and families on therapy methods, while ensuring HIPAA privacy. Experience in geriatric care is preferred.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must have a valid PT license and state registration.
• Experience in long-term or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess medical condition and functional capabilities.
• Establish and administer a treatment program with goals per physician guidance.
• Set realistic and achievable goals for patients.
• Document and educate patients and families in therapy methods.
• Perform evaluations, progress notes, recertification and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Maintain confidentiality, HIPAA and Resident Rights.
