Job summary

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• Licensure and Registration in Arizona; CPR.
• Six months of experience preferred.

Responsibilities

• Assess patient needs for physical therapy in a timely fashion; completes initial evaluation in accordance with department policies.
• Develops and maintains long and short-term treatment goals that are functional and measurable for each patient; reevaluates and modifies plans as warranted.
• Incorporates information, involves patient/family in planning; educates patients, caregivers, and other staff on status and recommendations.
• Utilizes the most appropriate form of treatment with the highest level of care.
• Maintains and upgrades professional skills through memberships, workshops, self-study, and inservices.
• Performs other related tasks or responsibilities as assigned.
