Job summary

Dartmouth Health Home Care is hiring a Patient Care Assistant to join our Home Care team in the Upper Valley region of Vermont. The role is full-time, daytime hours, providing essential non-medical support to clients in their homes, including daily living tasks and community mobility.

Ideal candidates will have a high school diploma and be willing to learn how to use iPhone-based apps required by state programs. Prior experience with elderly or disabled care is preferred but not required.

Qualifications

• High school diploma or equivalent is required.
• Willingness to learn to use iPhone and required apps.
• Preferred: experience in personal care of disabled/elderly; ability to use iPhone is a plus.

Responsibilities

• Provide non-medical, hands-on client care such as homemaking and activities of daily living.
• Assist with meal planning, grooming, toileting, feeding, dressing, and transportation to support independence.
• Record activities and maintain client-related paperwork; notify supervisor of changes.
• Support medication cues and adhere to infection control and confidentiality policies.
• Work under supervision in home care settings within the assigned service area.
