Job summary

Dartmouth Health Home Care is hiring a Personal Care Assistant for day shift per diem in the Upper Valley region. You will work closely with a care team to support patients at home, performing non-medical duties and daily living activities to maintain independence.

Requirements include a high school diploma, willingness to learn iPhone apps, and BLS within 90 days. Per diem status offers flexibility and a supportive community environment.

Qualifications

  • High school diploma or equivalent required.
  • Willingness to learn to use iPhone and apps required by DHHC and the states of Vermont and New Hampshire.
  • BLS certification within 90 days of hire; valid driver’s license with clean driving record; current car insurance.

Responsibilities

  • Provide non-medical, hands-on patient care including homemaking and ADLs.
  • Support client independence and safety under supervision per Plan of Care.
  • Prepare meals, groceries, and safe food handling.
  • Clean living areas (kitchen, bathroom, bedroom, living room).
  • Run errands for the client and cue medications without administering.
  • Inform supervisor of changes in client condition or home environment.
  • Record activities and submit required paperwork timely.

Responsibilities

The Patient Care Assistant Provides non-medical services, hands-on client contact including but not limited to: homemaking, helping with activities of daily living such as meal plan preparation, grooming, toileting, eating, dressing, reminding clients to take medications, and transportation in order to support client independence and safety so they may remain in their home.

  1. Under the supervision of the Long-Term Care Services Manager the PCA provides direct care to the client as outlined in the Plan of Care.
  2. Preparing meals; grocery shopping, storage, general cooking, storing in containers, utilizing safe food handling standards.
  3. Cleaning living areas including kitchen, bathroom, bedroom and sitting areas; includes the possible use of vacuum cleaners, brooms, general household cleaners, general household trash removal.
  4. Completing errands for the client.
  5. Medication cueing without administering.
  6. Informs the Long-Term Care Services Manager of any changes in the client's condition or home environment.
  7. Records activities and submits client-related and administrative paperwork on a timely basis, i.e., time sheets, errand sheets, or other documentation.
  8. Adheres to all policies and procedures including the infection control policy and client rights and responsibilities.
  9. Understands and maintains confidentiality.

Qualifications
Required Qualifications
  • High school graduate or equivalent
  • Must be willing to learn to use iPhone and apps required by DHHC and the states of Vermont and New Hampshire.
Preferred Qualifications
  • Experience with personal care of the disabled and/or elderly preferred.
  • Ability to use iPhone preferred.
Required Licensure/Certifications
  • Valid driver’s license with a clean driving record- Current car insurance, which meets minimum standards- BLS within 90 days of hire
  • Area of Interest: Service
  • Pay Range: $17.00/Hr. - $26.35/Hr.
