• Monitor and report changes in clients' health and behavior to the care team.
• Assist with mobility and transportation needs, ensuring safety.
• Provide medication reminders and maintain a clean, safe environment.
• Document care provided and update care plans as necessary.
• Communicate effectively with clients, families, and the office.
