Job summary

Care Guide Partners PACE is a part of Hosparus Health, a provider helping seniors 55+ stay in their homes with necessary medical care and support. The Personal Care Aide (PCA) performs in-home personal care and supportive services under the plan of care, collaborating with the IDT to ensure patient-centered care.

The PCA role emphasizes safety, comfort, and independence, assisting with daily living activities and reporting any changes to clinical staff.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years old.
• One year home health aide experience preferred.
• Experience with elderly population preferred.
• CPR certification required.
• Must be a licensed driver.
• Knowledge of safety and infection control in healthcare.
• Experience with EMR/electronic records preferred.
• Able to communicate effectively with adults and elderly.
• Able to operate medical equipment.

Responsibilities

• Provide personal care and supportive services to participants.
• Work primarily in participants' homes; may use PACE center.
• Maintain a safe, clean environment; observe changes and report to clinical staff.
• Collaborate with Interdisciplinary Team per plan of care.
• Assist with activities of daily living.
