Responsibilities

• Implements plan through coordination of care with the care team.
• Collaborates with the interdisciplinary team to achieve outcomes.
• Communicates patient data promptly to the appropriate staff.
• Promotes infection prevention and standard precautions.
• Assists with activities of daily living and records vital signs.
• Documentation of interventions and observations for patient care.
• Supports professional practice and ethics within Stormont Vail Health.
