Job summary

Hebrew SeniorLife is seeking a Patient Care Associate to protect, sustain, and nurture residents in a home-like setting. The role supports daily living activities, patient environment care, and coordination with the nursing team to ensure high quality care.

The PCA will work as part of a team, emphasizing patient dignity, independence, and safety. Ongoing training and strict confidentiality are expected in this on-site position, with compensation aligned to market rates.

Qualifications

• Fluent English skills required – verbal and written.
• Computer literacy required.
• Flexibility and ability to work independently and collaboratively.
• Must complete mandatory HSL/HRC educational requirements and programs.

Responsibilities

• Recognize and respond to the needs of patients and families.
• Communicate changes in patient condition to the nursing team.
• Adhere to Safe Patient Handling practices and maintain a no manual lift environment.
• Participate in Quality Improvement programs.
• Assist patients with personal care per care plans.
• Document care delivered in the electronic medical record.
• Report accidents, incidents, and observed abuse immediately.
• Observe and report skin integrity and patient status.
• Attend daily report and care planning meetings.
• Obtain vitals and basic procedures (temp, pulse, respirations, BP, weight, urine/stool) as directed.
• Follow infection control practices and participate in trainings.
• Maintain patient confidentiality per HIPAA regulations.
• Facilitate life enrichment activities and patient engagement.
• Maintain cleanliness of patient rooms and common areas.
• Prepare, serve, and cleanup meals and snacks.
• Participate in committee meetings.
• Perform other duties as assigned.
