Job summary

Hebrew SeniorLife is seeking a Patient Care Associate to support residents in a home-like setting. The PCA will assist with daily living, document care in the EMR, and collaborate with the nursing team to promote high quality life for patients.

Requirements include CNA certification or active nursing studies, fluent English, and computer literacy. This is an on-site role in the Boston area with comprehensive infection control and patient safety responsibilities.

Qualifications

• CNA certification preferred or current enrollment in nursing school.
• Fluent English required – verbal and written.
• Computer literacy required.

Responsibilities

• Recognize and respond to patient and family needs.
• Communicate changes in patient condition to the nursing team.
• Assist with activities of daily living and environment care as outlined in care plans.
• Document care delivered in the patient’s EMR.
• Maintain infection control practices and patient safety.
• Assist with meals, hygiene, and room cleanliness.
• Participate in daily report and care planning meetings.
• Observe and report skin integrity and any abuse concerns.
• Support patient engagement and life enrichment activities.
