Avenues Home Care, formerly Sentinel Homecare, is seeking a Patient Care Aide to assist clients with daily living activities, mobility, and personal care. You will document care and coordinate with nurses and supervisors to ensure high-quality service.

Position offers Full and Part Time options with 8/10/12 hour shifts, competitive benefits, and opportunities for mentorship. Prior caregiving experience preferred; CPR/First Aid and a PCA/CNA license are required.

Qualifications

• Proven ability to communicate clearly with clients and supervisors.
• Strong interpersonal skills and diplomacy in daily care.
• Ability to follow care plans and keep accurate logs.
• Maintain confidentiality of client information and comply with HIPAA.
• Must be fit for duty and maintain reliable transportation.

Responsibilities

• Assist clients with activities of daily living per care plan.
• Provide medication assistance, housekeeping, meal prep, and transportation as needed.
• Report daily activities and changes to supervisor; log accurately.
• Adhere to company safety policies and care standards.
