Responsibilities

• Greets and arrives patients; secures signatures for all required documentation; follows CHMG policies.
• Obtains and verifies insurance and demographic information; makes collection attempts and educates on resources.
• Facilitates care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information and maintains medical records.
• Processes and routes calls; responds to inquiries.
• Schedules, confirms or reschedules patient appointments and promotes enrollment in Patient Connect.
• Distributes mail/faxes/ electronic items; educates patients on form completion.
• Supports a patient-centered medical home culture; works independently or in teams on projects.
