Job summary

Big Sky Home Health of Bozeman is seeking an Advanced Practice Palliative Provider to perform comprehensive home-based palliative assessments and coordinate care plans in Bozeman, MT. This full-time, on-site role collaborates with physicians, nurses, social workers, and other professionals to support patients with serious illness and end-of-life care.

The ideal candidate holds a Master’s in Nursing or related APR field with palliative care training, Montana APRN licensure, and BLS/ACLS

Qualifications

• Palliative care experience with chronic and life-limiting conditions.
• Experience in primary care settings coordinating services.
• Background in critical or emergency care with acute symptom management.
• Current Montana license, BLS/ACLS, and prescriptive authority.

Responsibilities

• Conduct comprehensive palliative assessments in patients' homes and coordinate care plans.
• Collaborate with physicians, nurses, social workers, and other interdisciplinary team members.
• Educate patients and families about disease progression, treatment options, and goals of care.
• Document clinical encounters and maintain regulatory and quality standards.
• Participate in case reviews to improve patient outcomes.
