Role Description

The Advanced Practice Palliative Provider is a full-time, on-site role located in Bozeman, MT. This role involves conducting comprehensive palliative assessments, managing complex symptoms, and coordinating care plans that align with patients’ goals and values in their home. Daily responsibilities include collaborating with physicians, nursing staff, social workers, and other interdisciplinary team members to support patients with serious or life-limiting illnesses. The provider will educate patients and families about disease progression, treatment options, and supportive services, while facilitating advance care planning and shared decision-making. Additional duties include documenting clinical encounters, maintaining regulatory and quality standards, and participating in case reviews to continually improve patient outcomes.

Qualifications

• Strong foundation in Palliative Care, with the ability to manage complex, chronic, and life-limiting conditions.
• Experience in Primary Care settings, providing longitudinal, patient-centered management and coordination of services.
• Background in Critical Care Medicine and/or Emergency Medicine, including comfort with acute symptom management and clinical decision-making.
• Licensure and certification as a Nurse Practitioner or Advanced Practice Provider, with prescriptive authority in the state of Montana.
• Demonstrated skills in communication, empathic listening, and supporting patients and families through serious illness and end-of-life care.
• Ability to work collaboratively within an interdisciplinary team and to function independently in home and community settings.
• Master’s degree or higher in Nursing or related advanced practice field; completion of palliative care training or certification is preferred.
• Current state license, clean professional record, BLS/ACLS certification, and valid driver’s license for community-based patient visits.
